Vision Care Coordinator

SummaryAJANTA, LLCBethesda, MDThe Vision Care Coordinator will provide Care Coordination to the Vision Center of Excellence and established MTF's.
Responsibilities Monitor and track the treatment and regional care coordination of significant eye injuries sustained by Service members Assist with facilitating the coordination of ocular services between the Department of Defense (DoD) and Veteran Affairs (VA) medical treatment facilities, care providers, and case managers.
Identify Active-Duty Service members (ADSMs) with serious eye injuries and conditions that require vision care services coordination.
Collaborate closely with personnel in various healthcare platforms to enhance patient movement and transition between regions, facilities, agencies, and healthcare systems.
Provide local VCSC services to identified DoD SMs (and other beneficiary groups as time permits) with serious ocular injuries/conditions managed and treated at DoD facilities within the assigned local area.

Document and deliver vision care services coordination activities to the primary ocular/vision provider and other associated caregivers.
Communicate, in person or electronically, with DHA Market Medical Management Leads to share VCSC roles/responsibilities and vision care services and processes with case managers (on a scheduled or as-needed basis).
Collaborate with local and regional clinical communities focused on vision, surgical, medical, and trauma care to support the ocular/vision needs of injured ADSMs and Veterans treated at DoD facilities.
Liaise with various DoD and Service organizations caring and providing support for wounded Service members to advance awareness of eye injury patient needs and available resources (e.
g.
, Warrior in Transition program).
Provide weekly status reports that inspire as well as capture progress and issues Attend briefings, conferences, and other Government designated meetings as required in the execution of this effort or as requested by the COR.
Other duties as assigned Qualifications Possess and maintain an active unrestricted Registered Nurse license.
Bachelor of Science degree in Nursing (BSN) Minimum four (4) years of relevant experience in acute care/case management.
Acute care experience is defined as includes, intensive care, emergency room, trauma care, and operating room clinical experience.
Knowledge, Skills and Abilities Working knowledge of Microsoft Office, including Word, Excel, and PowerPoint.
Demonstrated experience in supporting research, complex efforts, multi-disciplinary teams to achieve desired outcomes and deliverables for Military Medicine/Defense Health Agency, or other agencies related to healthcare.
Ability to write, edit, and present reports.
